胃腺癌组织KAI1的表达及与血管、淋巴管生成的关系

The Expression of KAI1 in Gastric Adenocarcinoma and Relationship with Angiogenesis/Lymphangiogenesis

  • 摘要: 目的 寻找能预测胃腺癌(gastric adenocarcinoma,GAC)浸润、转移及预后的指标。方法 采用免疫组织化学法检测145例胃腺癌和50例癌旁正常胃黏膜组织抗癌1号(KAI1)、CD34及D2-40的表达。通过CD34的阳性表达计数微血管密度(microvessel density,MVD),通过D2-40的阳性表达计数淋巴管密度(lymph vessel density,LVD)。分析KAI1、MVD及LVD之间的相互关系,以及它们与胃腺癌浸润、转移及预后的关系。结果 在癌旁正常胃黏膜组织中,KAI1的阳性表达率、MVD和LVD分别为92.0%、 (9.2±7.8)/低倍镜(LP)和 (7.5±7.6)/LP;在GAC组织中,KAI1、MVD和LVD分别为37.2%、 (21.6±9.1)/LP和 (22.6±12.7)/LP,两组之间差异均有统计学意义(P<0.05)。KAI1的表达水平、MVD及LVD的高低与肿瘤细胞的pTNM分期、浸润深度、淋巴结转移与否有关(P<0.05);KAI1的表达与MVD及LVD呈负相关,MVD与LVD呈正相关(P均<0.05)。Cox回归分析示KAI1的表达、MVD及pTNM分期是影响GAC患者术后生存时间的独立因素。Kaplan-Meier生存分析表明,MVD≥22组和LVD≥23组患者其术后5年生存率低于MVD<22组和LVD<23组的患者(P<0.01);KAI1表达阳性的患者其术后5年生存率高于阴性患者(P<0.01)。结论 早期联合检测KAI1、CD34及D2-40对GAC的浸润、转移及预后判断有重要意义。  
    Abstract: Objective?To seek good markers to predict invasion and metastasis of gastric adenocarcinoma (GAC). Methods?Expression of Kangai 1 (KAI1), CD34, and D2-40 were examined by immunohistochemistry containing 145 specimens of GAC and 50 specimens of normal gastric tissue. Microvessel density (MVD) and lymph vessel density (LVD) were determined by the mean number of small CD34-positive or D2-40-positive vessels counted. And the relationship of KAI1, MVD and LVD, as well as the role of them on invasion, metastasis and prognosis in GAC were analyzed. Results?The positive rate of KAI1, the median scores of MVD and LVD in normal gastric tissue and GAC tissue were 92.0%, (9.2±7.8)/LP, (7.5±7.6)/LP and 37.2%, (21.6±9.1)/LP, (22.6±12.7)/LP, respectively. And there was a significant different between the two groups (P<0.05). The expression of KAI1, the scores of MVD and LVD were significantly related with pathologic-tumor-node-metastasis (pTNM) stages, depth of invation and lymph node metastasis (all P<0.05). There was a significant relationship between the expression of KAI1 and the scores of MVD or LVD. The survival rate of the KAI1 -positive or KAI1-negative group was significantly different (P<0.01); the survival rates were significantly lower in MVD≥22's group than that in MVD<22's group, so was the same relationship between the LVD≥23's group and the LVD <23's group (both P<0.01). Cox regression analysis: pTNM stage, expression of KAI1, and the scores of MVD were independent factors of postoperative survival time in GAC (P<0.05). Conclusion?The combined detection of KAI1, CD34, and D2-40 has an important role in predicting the progression and prognosis of GAC.
